Dr. Frank offers appointments from 9:30 AM – 8:00 PM on weekdays. Sunday morning appointments are available for emergencies only.

Once you have an appointment you
are encouraged to download and complete the intake and consent forms
prior to coming in for your first session. This will save time and
allow you to read the forms at your leisure. If
you want to allow Dr. Frank to speak to someone about you or your
child, then please complete the release form.
Forms
Intake-1: for children and adolescents 17 years old and younger
Description: basic information about school, family and social
functioning plus goals for therapy
Intake-2: for adults age 18 and older
Description: basic information plus goals for therapy
Consent: for adults and parents of minors
Description: allows Dr. Frank to provide therapeutic services and
includes information pertaining to therapy,
record keeping, confidentiality, relevant legal
matters, emergency contacts and billing
Release: for children, adolescents and adults
Description: allows Dr. Frank to speak to people in the client’s life (e.g. former therapist, medical doctor, teacher,
tutor, etc.)
